Fill out the required form fields on pages 2-10 of the report PDF and save to your computer again when completed with your organization name in the filename.
Fill in the contact info sections of the web submission form and attach your completed Progress and Use of Funds Report PDF.
You must include the following PDF attachments with your Progress and Use of Funds Report:

If your information was successfully submitted, you will be redirected to a submission confirmation page.
Since you received a grant from The Dana Brown Charitable Trust, you are be required to complete a end-of-year Progress Report within 10 months of the initial notice you’ve been awarded a grant.Deadlines for progress reports are as follows: If you received a grant during our Spring grant cycle (June), your progress report is due by 11:59 pm CST on December 15th. If you received a grant during our Fall grant cycle (December), your progress report is due by 11:59pm CST on June 15th.
